Finding 2 of 5 · The attrition leak. The client is a 200-person services organisation. Two engagement surveys and a salary benchmark had changed nothing, and exit interviews kept producing anecdotes. We mapped exit data by team and tenure. Departures clustered under three team leads, with an average tenure of 11 months on the floor. Pay wasn’t the driver. The driver was one handoff: the shift handover, where a new joiner’s first questions were met with delay and visible irritation. New joiners learned to work in silence, went months without recognition, and left the moment the market offered acknowledgment.

Recommended action. One ritual, fifteen minutes a week, installed at the team level with Poka-Yoke: a visible tracker that made skipping obvious. Attrition fell from 34% to 21% within two quarters.

You should know exactly what an investigation commits to before you spend anything. So here it is, plainly.

What an engagement commits to

Findings you can act on: the Pattern Report, the Leak Map, and one high-impact recommended action. Remediation support where the written scope includes it. Every finding stated so it can be proven wrong, with the evidence behind it. Fixed price, fixed timeline, agreed in writing before work begins.

What it doesn’t commit to

Legal representation or legal advice; we are investigators, not advocates. Staffing changes, placements, or headcount decisions; those stay with you. Guaranteed business outcomes; attrition, defect rates, and compliance behavior move for many reasons, and no honest investigator will promise you a number.
